Full-time Practice Nurse Job near Bridgwater, Somerset

An excellent practice near Bridgwater, West Somerset is seeking a new Practice Nurse to join its team.

This innovative practice is looking to expand its nurse team with an experienced Practice Nurse. The practice prides itself on exceptional care for its patients which are delivered by the multi-disciplinary team, who are there to support you. They use modern care systems to maintain their high standards, making this a great opportunity for a nurse seeking a new challenge.



Excellent organisation throughout the practice means that you are able to focus on delivering the highest quality of care whilst having a manageable workload.





Key Duties



Your duties will include utilising the triage system, with some face to face appointments. Your clinics will include Chronic Disease Management (Diabetes and/or hypertension) and screening. You will be supported by the lead nurse and accountable clinically to the GP team.



Hours are flexible, with no out of hours.

Key Skills, Qualifications and Client Requirements

• Registered General Nurse essential
• Primary care experience essential
• Chronic disease management: Diabetes and Hypertension desirable
