EXTENSIVIST GENERAL PRACTITIONER/ CONSULTANT EXTENSIVIST
Salary range – Circa £80k to £100k (depending on experience)
Full time (40 hours) and/or Part time hours considered
(Part time post holders must be available to work a minimum of 2 days per week)
The Role and the Service
We are seeking to recruit a General Practitioner(s) with an interest in Frailty and co-morbidity to join our other GPs already in post in the Tameside and Glossop Integrated Care Foundation Trust - Extensive Care Service.
The Extensive Care Service is a fundamental part of the Trust’s ambition to improve care for those residents living with a number of long term conditions and associated health and social care issues, to improve their health and wellbeing, and reduce demand on services by ensuring that their care is managed more effectively in the community. We are working innovatively with our stakeholders on new models of care within Tameside and Glossop, this is an ambitious transformation programme. It is designed to ensure that health and social care services for the people of the Tameside & Glossop are integrated to provide better care outside of hospital.
This is an exciting role which will offer the successful candidate the opportunity to innovate and shape the service as it continues to develop.
Your strengths
- You will be a doctor with a passion for service development and improvement, who will be able to bring your clinical expertise and experience in managing patients with multiple long term conditions.
- You will be able to demonstrate positive clinical leadership within a multi-disciplinary team, made up of Advanced Nurse Practitioners and other clinicians, working closely with colleagues in neighbourhood pharmacy teams and social prescribing projects.
- You will be capable of forging good working relationships with local GPs, as well as hospital clinicians in the acute Trust. It is an exciting opportunity to take a leading role in a service that aims to provide a clinical interface between primary, community and secondary care.
Your Part in the Transformation Plan
The Tameside and Glossop Locality Plan set the bold ambition of raising healthy life expectancy to the North-west average by 2020. Our vision to achieve this ambition is to move quickly to a fully person-centred and integrated model of care, with a much heavier emphasis on prevention, supporting self-care and care closer to home.
We are one of the first Integrated NHS care organisations to develop Integrated Neighbourhood teams across our five Neighbourhoods; each has access to support from a range of more specialised community-based services. These include intermediate tier services such as community inpatient and discharge to assess beds, podiatry, speech and language therapies, IV therapies, community rehabilitation and dietetics, as well was some urgent care services.
The intermediate tier will provide a key interface between the Integrated Neighbourhood and the acute hospital setting ensuring that people are supported in the environment that is suited to their own needs, avoiding unplanned admissions and supporting patients to return home after hospital care. The Extensive Care Service is a key development within the Intermediate Tier.
We can offer an exciting opportunity to work in a health and care system that has excellent relationships between Local Authority, CCG, Trust and Primary Care and who are all committed to improving the health and wellbeing of the population and creating a health and care system that is sustainable, within a new model of care.
